Caesarian sections in the U.S. hit a record high for the eleventh year in a row –averaging about one-third of births nationally, up from 26 percent in 2002 and 5 percent in 1970. The rate is 36 percent in New York (45 percent at Albany Medical Center) and 39 percent in Florida (50 percent in Miami Dade County).
Some argue the high statistics are due to older, overweight mothers, larger babies, and multiple births, but statistics don’t validate this. Another reason is the risk of malpractice claims prompting doctors to make decisions for non-medical reasons. C-section rates are higher for women who have private medical insurance and deliver in a private hospital, suggesting financial incentives may play a role. Doctors now perform caesarean sections rather than use forceps or deliver a baby in breech position (feet first). Predicting the due date is not an exact science, and doctors worry if a baby is overdue. They’re anxious and they’re making their already anxious patients unnecessarily fearful about nature’s natural process – a vaginal birth. Whether because of doctor’s anxieties or hospital and doctors’ schedules, doctors often induce labor and/or recommend C-sections. A troubling and prevalent reason derives from the increased use of technology and use of labor inducing drugs. One example was a caesarean on a woman whose ultrasound showed the fetus to appear larger (12 lbs. 10 oz.) than it actually was (9 lbs. 4 oz.). Ultrasound is not a reliable method for determining the baby’s weight. In another case, a young woman believes that the crowded hospital facilities in New York City prompted her doctor to induce labor. A Belgian study revealed that induced labors involves significantly more pain medication and leads to more cesarean births due to both fetal distress and stalled labors. Once there is induction, the baby must be monitored. The drugs administered to this New York mother affected the baby’s heartbeat (fetal distress), necessitating a C-section. There is also a risk that the baby will be in the wrong position because it’s not ready to be born, or that the induction will not work. If the mother’s water has broken, the baby must be born by C-section for its safety.
Some doctors base their decision on a standardized labor curve developed by Emanuel Friedman forty years ago. Newer research has shown a “wide range of normal,” with a curve that differed markedly from the Friedman curve. (“Reassessing the Labor Curve in Nulliparous Women,” by Zhang, Troendle, &Yancey, American Journal of Obstetrics and Gynecology, 10/2002, Vol. 187, No. 4) One doctor at Cedars-Sinai Hospital in Los Angeles told his patient (who requested anonymity), “If you’re not progressing in labor according to the Friedman scale, we have to do what’s best for the baby,” and, “If you’re not dilated at a certain point, we have to intervene.” When asked if there were any possible side effects from a C-section, he replied, “Absolutely not.” The patient told me that he made it seem as if natural childbirth was a risk. This mother did some investigation on her own. She learned that the Cedars’ rate of C-sections was 20 percent higher among PPO patients than HMO patients. She ultimately elected to have a natural delivery at a birthing center.

The leading cause is that elective caesareans increasingly are being performed late pre-term (34-37 weeks). A survey revealed 36 percent of women who had elected to have a second C-section (before entering labor), elected to deliver before 39 weeks. Another reason electing a C-section prior to onset of labor is that due dates are often miscalculated, and doctors think the baby is overdue. Babies delivered by C-section at 37 weeks are twice as likely to experience bloodstream infections and other complications, and have a four times greater risk of breathing problems. Those born just one week shy of the recommended 39 weeks of gestation have a 50 percent greater chance of suffering such problems. (“Trend to C-Section Births,” supra) Interestingly, one survey revealed female women doctors perform fewer patient-requested caesareans.
In contrast, at a small Navajo hospital in Tuba City, only 13.5 percent of first-time mothers have caesareans, and 32 percent of mothers have vaginal birth after caesarean (VBAC) – over three times the national average. The reason is that nurse-midwives deliver most babies, with a doctor on-call if necessary. Midwives are more patient and specialize in coaching women through labor and are less likely to induce it before the due date. They don’t have to rush home to a golf game. Of her prior job in North Carolina, Nurse Rackley at the Navajo hospital said that if doctors didn’t want to work late, they’d set an arbitrary deadline for delivery, and if nature didn’t meet it, the mother had to have a caesarean. Another difference in Tuba is that the staff doctors are paid and insured by the federal government, so there’s no financial incentive to perform C-sections, nor are VBAC’s banned as they are by some hospitals or insurers who either deny coverage or charge much higher premiums. As the use of midwives at home births and birthing centers is growing, the American Medical Association has begun lobbying Congress and each state to eliminate deliveries by midwives requiring hospital births by doctors.
The March of Dimes is campaigning to empower women with information so that they can have informed discussions with their obstetricians well before delivery. This is also my purpose. There are cases where caesarean is the wisest and safest choice for both mother and child, but expectant mothers should be informed of their options, question their doctors and feel comfortable with a mutually agreed-upon plan long before labor begins. My concern is that decisions are being made for women that aren’t in their best interest, and that technology is alienating us from their own bodies. Women need to be reminded that our bodies are designed to deliver a baby – to conceive it, grow it, and birth it when it is ready to survive outside the womb.

Obtains continuing education on coding each year [at the practice's expense] with close attention to changes affecting the practice’s specialty;Â
Trains appropriate staff on coding matters, including changes that affect the practice and its specialty each year; and
Provides formal coding training sessions for new clinicians and new billing staff members within 30 days of hire.
With physicians typically charging a minimum of 0,000 a year [and double that for some specialties], it’s worth protecting your revenue and investing in a resident coder to keep the practice on track with billing properly for the services that are rendered. There’s a big upside to having a sharp coder that helps the entire office understand coding requirements so that you get paid better for what you do.
Once billing is accurately submitted, the arduous job of following up on claims begins.  Following up on claims is where staff expertise pays off big time!
Auditing claims payment and sending appeals is vital to protect the practice’s revenue. Don’t assume the insurance plans are adjudicating claims appropriately. They make errors that result in lower reimbursement for the practice.Â
It’s difficult to imagine, but in the United States a whopping 30% of insurance claims submitted are denied, according to Healthcare Business Advisors, LLC, Albany NY 2007 report – and of that 15% are resubmitted. Guess who gets the short end of that stick? You are right – It’s the practice.
CMS (Center for Medicare Services) reports that Medicare denies 11% of submitted claims and 40% of those are never resubmitted. That’s a huge number. To make it even more interesting, Medicare data reveals 65% of the claims reviewed on appeal result in increased payments. Experts say 50-80% of appealed claims are eventually paid – so fight for your money!Â
The message is clear: Hire the best, let them know what you expect and treat them right. This presents new challenges, as some medical office staff members abandon the practice environment and seek opportunities outside of medicine. To identify ways to appeal to the best candidates, answer a few important questions:
How attractive is our practice opportunity for a prospective employee;
What significant changes can be made to appeal to the best candidates; and
Does our culture value staff?
A culture of respect places a high value on staff – respecting them as individuals and for their talents – and it starts top-down. Practice leaders must demonstrate how they value employees in their words and actions. Don’t compromise staff’s value by cancelling staff meetings, allowing physicians’ failure to participate in staff meetings, or by failing to respond to their job needs or delaying purchasing needed equipment so they have tools to do the job better and grow their skills.
Communication is essential to building strong relationships with staff. Make sure staff members understand what you expect from them and provide the guidance to help them meet that expectation. When staff members feel appreciated and know they have management’s support, they will band together to reach higher levels of success.
Healthcare reform brings many new issues to the forefront; some are obvious, others are fluid and will change over time. Guidelines and regulations will continually change. Those regulations will impact the way physicians conduct their business, the revenue they generate and the overall profits that are attainable.
One thing is evident; physicians are being pushed into the information age and the need to embrace electronic health records to manage internal data and provide clinical data to regulatory agents that will be used for industry experts to define standards of care and quality of services.Â
It is the responsibility of physicians (and the administrative staff they depend on) to be well-informed and make prudent decisions in the interest of providing a high level of service and use wise economic practices. Keeping everyone in the office informed about their sphere of influence in both the service and financial component of the practice is just plain smart. Good communication and a clear sense of what it takes to succeed keeps everyone working as a team with a vested interest in achieving practice goals and rising above the nuances of practicing medicine in times of reform.
Your continual challenge will be to improve revenue and manage costs without compromising patient care or services. Methods to achieve this vary, including:
Advancing the quality of internal systems and services;
Establishing and monitoring clear standards of care;
Identifying when it is time to outsource and tap into another level of expertise;Â
Streamlining processes and eliminate those without value; and
Obtaining optimal performance by achieving high morale and impressive productivity.
In the end, the most important factor is creating a culture of value and respect across the continuum of care and across the organization – a culture where everyone wins!

Protect yourself Before and After a Car Accident
Protect yourself BEFORE you have an accident by keeping in your glove compartment several items, such as: an emergency kit, a pen and paper for taking notes, a disposable camera for taking photos of the vehicles at the scene, and a card that lists your medical allergies or conditions just in case there are serious injuries. You should also have a set of cones, emergency flares or warning triangles in the trunk. Of course, drivers should carry a cell phone as well.
1. Before you have an accident, keep an emergency kit in your glove compartment. Drivers should carry a cell phone,as well as a pen and paper for taking notes, a disposable camera to take photos of the vehicles at the scene, and a card with information about medical allergies or conditions that may require special attention if there are serious injuries. A set of cones, warning triangles or emergency flares should be kept in the trunk.
2. If you have an accident: Stop. Do not drive away from the scene! You can be charged with “hit and run” even if the accident wasn’t your fault if it involves a pedestrian, a moving car, or even a parked car or someone’s property.
3. Move vehicles to safety. Make every effort to prevent further accidents. You may be liable for damages to approaching vehicles unless they are properly warned. Do not move your car too far. Make sure to turn on hazard lights and set out cones, flares or warning triangles, if possible. If your car can’t be driven, have the emergency responders call a tow truck. Get the name, address and telephone number of the towing company and, if you can, find out where it is taking your car.
4. Move people to safety. Check for injuries. Everyone who can walk should move to safety.
5. Call for help. As soon as you can get to a telephone, call 911. Explain the situation and give the exact location of the accident, so that help can arrive quickly. Be sure to mention whether you need an ambulance or a fire engine. Don’t hang up until the operator tells you it is okay to.
6. Cooperate with the authorities. In New York State if you are involved in a vehicle accident causing injury to person or property, you are required to stop and give your name, address, and show your driver’s license and insurance identification card to the person who is injured or whose property is damaged, or to a police officer. If such information isn’t available at the scene of the accident, it must be given to the nearest police station or judicial officer as soon as possible. Be sure to cooperate with the police officer investigating the case. But,stick to the facts. For instance, if you were driving 30 miles an hour, say so. Do not say, “I wasn’t speeding.”

7. Tell the police the truth. Follow police instructions carefully. Note the officer’s name and precinct number, command, or barracks. Ask how you can get a copy of the police accident report.
8. But don’t admit fault. Do not volunteer any information about who was to blame for the accident. You may think you are in the wrong and then learn that the other driver is as much or more to blame than you are. You should first talk to your insurance agent, your lawyer, or both. Anything you say to the police or the other driver can be used against you later. Do not agree to pay for damages or sign any paper except a traffic ticket until you check with your insurance company or lawyer.
9. Document what happened. Make a note of the details of the accident, including date and time, road conditions, weather conditions and speed of all other cars involved. It also is a good idea to draw a diagram of the accident showing the position and direction of the cars justprior to and after the accident: make a written description of each car, including year, make, model and color and the exact location of the collision and how it happened.
10. Take photographs. Use your camera to document the damage to all the vehicles. (Use a cell phone’s camera if you have nothing else.) Keep in mind that you want your photos to show the overall context of the accident. Take your own photographs even if the police take photographs or you believe that your mechanic or insurance company will take them.
Photograph: (a) any broken parts, inside or outside the vehicle such as dashboard, windshield, seat, broken glass or lenses, bumper that fell off, etc.; (b) vehicle license plates; (c) skid marks; (d) street signs; and, (e) injuries to persons involved in the accident. Take the photos from different angles and positions. Take more photographs than you think you’ll need. ALWAYS HAVE SOMEONE WATCH FOR TRAFFIC.
11. Exchange information. Exchange the following information after the accident: name, address, phone number, insurance company, policy number, driver license number and license plate number for the driver and the owner of each vehicle. If the driver’s name is different from the name of the insured, take down the name and address for each individual. If possible, obtain the names of the insurance companies and the policy numbers covering the other autos in the accident. By law, an insurance policy information card must be carried in every vehicle licensed in New York State. These cards can be your source for such information.
12. Get witness information. If there are witnesses, try to get their names, addresses and telephone numbers; they may be able to help you if the other drivers dispute your version of what happened. Request that they talk to the police before leaving. If they refuse to identify themselves, jot down the license plate numbers of their automobiles. Do not discuss the accident with the witnesses. Don’t share your witness information with anyone except the police, your attorney or your own insurance company.
13. Report to your insurance company. Remember to call your insurance company or insurance broker immediately after the accident. Don’t forget to send written notice to your insurance company as soon as possible. The notice should include the time, place and circumstances of the accident. If you fail to notify your insurance company of the accident in writing within a reasonable time, this could be grounds for the company’s denying any obligation to protect your interests in the event a claim is made against you. Also, make sure your report contains the names of all injured persons and drivers and the names of all available witnesses.
14. Report to Albany. The law requires that the operator of a vehicle involved in an accident in New York State in which a person is killed or injured or one in which damage to the property of any person, in a written report with the Commissioner of Motor Vehicles within 10 days. If the operator is unable to make such a report, another participant must make it. Remember to keep copies for your records. This report form, known as an MV-104, may be obtained from the Department of Motor Vehicles or its website (over the Internet). Often, your insurance company, insurance broker or attorney can send you the form.
15. Call an attorney. Consult an attorney who is experienced in accident cases if you or a loved one is hurt. Ask lots of questions.

New York is the heart of USA and also known for various trends. A lot of economy of USA is based on this state. It is the biggest state of USA too. New York’s gross state product in 2007 was .1 trillion, ranking third in size behind the larger states of California and Texas the other two states of USA. If New York were an independent nation it would rank as the 16th largest economy in the world. Its 2007 per capita personal income was ,364, placing it sixth in the nation behind Maryland and eighth in the world behind Ireland.
New York’s agricultural outputs are dairy products, cattle, other livestock, vegetables, nursery stock, and apples. Its industrial outputs are printing and publishing, scientific instruments, electric equipment, machinery, chemical products, and tourism. A recent review by the Center on Budget and Policy Priorities found 13 states, including several of the nation’s largest, face budget shortfalls for Financial Year 2009. New York faces a deficit that could be as large as .3 billion. New York exports a wide variety of goods such as foodstuffs, commodities, minerals, computers and electronics, cut diamonds, and automobile parts. In 2007, the state exported a total of .1 billion worth of goods, with the five largest foreign export markets being Canada ( billion), United Kingdom ( billion), Switzerland (.9 billion), Israel (.9 billion), and Hong Kong (.4 billion). New York’s largest imports are oil, gold, aluminum, natural gas, electricity, rough diamonds, and lumber. Canada is a very important economic partner for the state. 21% of the state’s total worldwide exports went to Canada in 2007. Tourism from the north is also a large part of the economy. Canadians spent US7 million in 2004 while visiting the state.
New York City is the center of banking, finance and communication in the United States and is the location of the New York Stock Exchange, the largest stock exchange in the world by dollar volume. Many of the world’s largest corporations are based in the city. The state also has a large manufacturing sector that includes printing and the production of garments, furs, railroad equipment and bus line vehicles. Many of these industries are concentrated in upstate regions. Albany and the Hudson Valley are major centers of nanotechnology and microchip manufacturing, while the Rochester area is important in photographic equipment and imaging.
New York is a major agricultural producer, ranking among the top five states for agricultural products such as dairy, apples, cherries, cabbage, potatoes, onions, maple syrup and many others. The state is the largest producer of cabbage in the U.S. The state has about a quarter of its land in farms and produced US.4 billion in agricultural products in 2001. The south shore of Lake Ontario provides the right mix of soils and microclimate for many apple, cherry, plum, pear and peach orchards. Apples are also grown in the Hudson Valley and near Lake Champlain. New York is the nation’s third-largest grape-producing state, behind California and second-largest wine producer by volume.
